Before your Visit

The better you prepare for your first office visit, the better I will be able to help you. One of the most important things you can do is take a few minutes and think about what you want to discuss such as:
- What your symptoms are
- When they started
- How you cope with them

Also, it may help to write down those questions you would like to ask but may not remember when you get in the office. In addition to the general information you will want to share with me, please be sure to bring the following with you on your first visit, if they apply to you:

- Your insurance card and social security card
- Necessary referrals
- Test results, x-rays, diagnostic/lab reports (if these have not been sent separately)
- The name, address, and phone number of your primary care physician
- Information about your family medical history
- Information about your personal medical history
- The person to contact in an emergency
- A list of the medications you are currently taking and other recent treatment history, eg, chemotherapy
- Your list of questions
- Something to record the answers to your questions, like a notepad or binder which could also be used to hold any information I may give you
- Someone who can come with you to hear the things you may not hear and to just be there for support.
